Intervention plan

Create and implement an intervention plan based on an individual child's unique interests, strengths, sociocultural context and next development steps/goals.

  1. Identify one of your focus children (identified with a disability), and gather  information about child's strengths, next developmental steps, family's priorities and concerns, select an outcome/short term objective within the IFSP or IEP (this should align with the focus child overview).
  2. Using this outcome, track 3-5 days of baseline data, using the Behavior Rating Scale in the PTR-YC book (or another data tracking tool used at your site).
  3. Create an Intervention Plan based in the child's daily routine at school or home. In a separate narrative, describe evidence (in literature) for the interventions selected (with references).
  4. Implement plan and track data for 6-10 days, using same form as above.
  5. In your narrative, describe any revisions or adaptations you would make based on the data collected, as well as how services were provided in the natural environment (infants/toddlers), and the least restrictive environment (preschoolers).
  6. Reflection:

Teaming: Discuss collaboration with the team- what went well? What would you do differently?

Instruction/Assessment: What did you learn about planning and implementing an intervention plan (or supporting classroom staff in implementing)? What did you learn about data collection and analysis?

Overall: What you would recommend as changes/ improvements to the way intervention plans are conducted at this practicum site based on DEC Recommended Practices?
